It's not something we like to talk about- how we are going to live in the last years of our lives.  For many of us, just starting the conversation is the biggest challenge.  We created this toolkit to help.  Whether it be finding information for yourself, for your loved ones,  for a patient or for your community, this website will get you started. 

The institute of medicine notes most people nearing the end of life are not physically, mentally, or cognitively able to make their own decisions about care. The majority of these people will receive acute hospital care from physicians who do not know them. Therefore, advance planning is essential to ensure that you receive care reflecting YOU.  It's never too early to be thinking about what is important to YOU.
